ℹ Please attach the following requirements for RSBSA registration:
  1. RSBSA Enrollment Form (this form, filled out and signed)
  2. Barangay Certification
  3. Photocopy of a valid ID
  4. Geotagging of the farm/farmland being tilled (captured above)

JPG, PNG, or PDF. If you filled out this form on paper first, upload the scanned/signed copy here. MAFAR staff can also print this form from the system once encoded.
